Within the first week of admission and periodically thereafter, all clinical departments — including nursing, social services, dietary and recreation — visit and assess each resident. All involved clinicians then meet and develop an individualized plan of care that details the specific needs of each resident. This interdisciplinary approach serves to address the resident’s medical needs, along with minute details such as preferred shower times, food preferences and recreational abilities and interests. Residents and family members are encouraged to partake in this meeting and to offer input!
